TB Infection  Referral Form for Applicants needing LTBI Treatment
Use this form when referring patients to community clinics for LTBI treatment. Return this form to the local health department after treatment is completed.
TB Disease  List of LAC Clinics that Provide LTBI Treatment
Civil Surgeons are encouraged to treat applicants with LTBI themselves. For information on treatment regimens, please see the Provider Guidance Toolkit. If an applicant has insurance they should be referred to their private provider for treatment. Clinic list available in Spanish.
