Complete this form in its entirety and submit to TB Control within one working day from the time of TB identification.

Complete this form in its entirety for hospitalized patients and submit to TB Control within one working day from the time of TB identification.

Patients suspected or confirmed with tuberculosis may not be discharged or transferred from a health facility without prior approval. Complete this form and submit to the TB Control Program 1 to 2 business days prior to the anticipated discharge.
