Communicable Disease Reporting in California

The Provider Reporting Portal is Now Online!

Beginning September 1, 2026, the Los Angeles County Department of Public Health (DPH) transitioned non-urgent mandatory communicable disease case reporting to the California Reportable Disease Information Exchange (CalREDIE) Provider Portal. This is a secure, online system to replace mail, email, and fax-based case reporting. This change does not apply to diseases that require immediate reporting, reporting on HIV, hospital discharge approvals for tuberculosis, or animal bites to humans. FAQs - General Questions

Does this change the mandated reporting requirements?

No, there is no change to case reporting requirements that are mandated by Title 17, Section 2500, including requirements for immediate reporting by telephone for select diseases or conditions.

Are health care providers still able to report by phone, email, or fax?

Yes. Mailed, faxed or emailed CMR submissions are still accepted, although the CalREDIE Provider Portal is preferred. Diseases that require immediate reporting should continue to be reported by phone.

How can I confirm that a report was successfully submitted?

A confirmation screen will appear upon submission. At any time, users can review a list of their submitted reports within the Provider Portal.

How do I attach supporting documents to my report?

To attach any supplemental documentation to your report, submit your report and then select “Add to Filing Cabinet” to upload files.

The condition that I am reporting is not a listed option. How should I proceed?

If it is not listed, select “Unusual/Other Disease” (e.g., nontuberculous mycobacteria [NTM]) and write the condition or disease in the free text field.

Is the Provider Portal the same as electronic case reporting (eCR)?

No. Electronic case reporting (eCR) is a separate method for submitting reportable disease information directly from a health care organization’s electronic health record (EHR) system to Public Health without manual intervention. Organizations that are connected via eCR are still required to continue manual case reporting (which includes the Provider Portal) until there are changes to mandated reporting requirements.

Does the transition to the Provider Portal affect electronic laboratory reporting (ELR)?

No. ELR is a separate reporting mechanism used by clinical laboratories to electronically submit reportable laboratory results to public health. This transition does not affect ELR and facilities should continue reporting via ELR as usual.

Who needs to register for a CalREDIE Provider Portal account?

Everyone who submits reports through the Provider Portal must have an individual user account.

Facilities should identify the individuals who are authorized to submit reports on their behalf and ensure that each designated reporter completes the Los Angeles County DPH CalREDIE Provider Portal Account Authorization Form. If the reporter submits reports for multiple facilities, they should add each facility to their account by completing the "additional location" portions of the Authorization Form.

Do clinicians need to submit their own reports?

No. Any staff can be designated to submit reports for clinicians and their facility.

If I already have a Provider Portal account for another jurisdiction, do I need to complete the Los Angeles County DPH CalREDIE Provider Portal Account Authorization form?

Yes, but only to add the appropriate Los Angeles County reporting location(s) to your account. There should only be one account per reporter.

Add appropriate facilities to your account by selecting the “Change Existing Account” for your “Account Action” in the Los Angeles County DPH CalREDIE Provider Portal Account Authorization form.

If I report for multiple facilities in Los Angeles County, do I need to create a separate account for each facility?

No. There should only be one account per individual. Each distinct facility location must be added to the user’s account using the “Change existing Account” option on the Los Angeles County DPH CalREDIE Provider Portal Account Authorization form. Once you receive confirmation from LAC_PPSupport@ph.lacounty.gov that your account has been created or updated, you can submit reports on behalf of multiple facilities.

Does each designated reporter need an individual account?

Yes. Users should not share login credentials. Each person who submits reports through the Provider Portal should use their own individual user account.

Is there a limit to the number of designated reporters a facility may have?

No. A facility may designate unlimited reporters to submit reports through the CalREDIE Provider Portal.

Our organization operates multiple facilities. Do I need to include each facility in my registration?

Yes, if you report for multiple facilities. While completing the Los Angeles County DPH CalREDIE Provider Portal Account Authorization form, you should add each facility to your account by completing the "additional location" portions of the Authorization Form.

Can the facility name be abbreviated or an acronym?

No. Please enter the facility’s full name rather than an abbreviation or acronym.

The Provider Portal is a secure, web-based interface that allows Confidential Morbidity Reports (CMRs) to be submitted electronically to CalREDIE.
