How to Make a Media Request

For all media inquiries, please email media@ph.lacounty.com.

Public Records Act (PRA) Requests

Members of the public can request records by filling out the CPRA Records Request Form and submitting it to phicor@ph.lacounty.gov.

You should receive a response within 10 days after your request is received. If you have any questions, you can contact the DPH Custodian of Records at (562) 345-3404 or by email at phicor@ph.lacounty.gov.

Media Requests

Members of the media should submit CPRA requests to media@ph.lacounty.com.

Death Certificate Submission

Informational death certificates may be obtained in person, by mail, or online. Our office is located at 313 N. Figueroa Street, Room L-1, Los Angeles, CA 90012 and is open Monday – Friday, 8:00 a.m. – 4:00 p.m.

Processing Time

Informational death certificates are redacted by the state and may take two weeks or longer to process once finalized.
